Before I have an IEP or parent meeting, I always get feedback from the student’s teacher or teachers. I get feedback on the student’s grades, work habits, and behavior. I used to email teachers asking for info but I would relay ever get feedback or on if I did it was super vague. When this happened I would have to go find the teacher to get current feedback. A few years passed and I went to a SPED conference where the presenter showed us her feedback form that she used as special ed teacher..and BOOM my mind was blown.
I re-created her form into what would be appropriate for high school students and middle school students. I started giving the form to my gen ed teachers and actually got them back!!! When I asked the teachers why they said the form was easy. I would email the form to teachers and then put one in their boxes too. I always used bright paper, so it was easy to remember and find on a busy desk. I have to admit that I did stop using the form when I started at a private school but after having the same issues again I decided to try it revamping it for my current students. Anyways, I wanted to share it with you guys too.
